Healthy Mom - Healthy BabyPhysicians Health Plan has entered into a partnership with physicians to promote education and individualized services to pregnant members, new moms and babies. The program is called Healthy Mom Healthy Baby.

The Healthy Mom Healthy Baby program encourages all pregnant members to: Make an appointment to see your physician within the first twelve weeks of your pregnancy; Make sure you have your screening lab work done, especially Hepatitis B screening; See your physician for your post partum appointment within 21-56 days after delivery; Set up your baby's visit upon discharge for immunization visits.

Services that PHP offers include:

  • Health Risk Appraisal Forms:
    Used to identify women at risk for difficult pregnancies.
  • Prenatal Vitamins:
    A 90 day supply of prenatal vitamins may now be obtained at one time. If you have a co-pay, you will only need to pay a one-month co-pay for the 90 day supply of vitamins. Any deductibles must first be met prior to receiving a 90 day supply for one co-pay.  This benefit is available if you have a prescription rider on your PHP policy. A special authorization must be obtained to receive this benefit.   • Health Promotion Information:
    Written material designed to answer specific questions about health and pregnancy.
  • Six-week Postpartum Incentive Form:
    If a member completes her postpartum visit between 21-56 days from delivery, and completes the incentive form, she is eligible for a free gift card.
  • Prenatal Class:
    Prenatal classes are offered locally through Expectant Parent Organizations. PHP will reimburse the provider for any of the classes that are listed in the Healthy Mom Healthy Baby packet.
  • Prenatal Visit Card:
    If a member has kept all of her scheduled prenatal visits, and completes a Prenatal Visit Card; she is eligible for a free gift card.
  • Enrollment in Program:
    PHP looks forward to working with you and your physician toward a successful pregnancy and healthy baby. To receive a packet about the Healthy Mom, Healthy Baby program, call 517.364.8466, or 1.877.803.2551 toll-free, then select option 4.

Warning!

Babies are dying because they are put to sleep in places that are not safe. Babies always need a safe sleep place.

Parents whose babies have died want other parents to know:

  • Babies are not safe sleeping on a couch, pillow, or anything soft.
  • Babies are not safe sleeping with other children, adults, or pets.
  • Adult beds are not safe for babies. If you feed your baby in bed, put your baby back into his/her crib to sleep.
