According to the Centers for Disease Control and Prevention, more than 64 million Americans have one or more types of cardiovascular (heart) disease. Many Americans have health conditions or make lifestyle choices that put them at risk for developing heart disease.
Physicians Health Plan (PHP) has entered into a partnership with members and physicians in achieving and maintaining the optimal cardiovascular health of our members. PHP encourages members to learn about their risk for heart disease and actively participate in preventative activities. People with risk factors or known heart disease can lead active, healthy lives.
The Healthy Heart Program helps members learn about and maintain a heart healthy lifestyle and supports members recovering from a cardiovascular event.
