Healthy HeartAccording to the Centers for Disease Control and Prevention, more than 64 million Americans have one or more types of cardiovascular (heart) disease.  Many Americans have health conditions or make lifestyle choices that put them at risk for developing heart disease.

Physicians Health Plan (PHP) has entered into a partnership with members and physicians in achieving and maintaining the optimal cardiovascular health of our members. PHP encourages members to learn about their risk for heart disease and actively participate in preventative activities.  People with risk factors or known heart disease can lead active, healthy lives.

The Healthy Heart Program helps members learn about and maintain a heart healthy lifestyle and supports members recovering from a cardiovascular event.

The program provides support and resources to encourage members to live heart healthy lives by:

  • Having regular blood pressure screening and controlling high blood pressure
  • Having regular cholesterol level checks and controlling high cholesterol levels
  • Quitting Smoking
  • Participating in physical activity
  • Eating a nutritious diet
  • Maintaining a healthy weight
Services Offered by the Healthy Heart Program
 

Health Promotion
The Healthy Heart Program provides:

  • Written materials for the member designed to answer specific questions about heart disease prevention and management.
  • Reminders for cardiovascular care opportunities such as blood pressure checks and cholesterol screening.
  • Seminars for members with risk factors or cardiovascular disease and their family members to provide education about heart healthy lifestyles.

Heart Disease Prevention and Management Education

The Healthy Heart Program provides:

  • Education about heart disease prevention and/or the management of cardiovascular disease to support the physician’s plan of care.
  • PHP guidelines for cardiovascular disease prevention and management (based on national cardiovascular care guidelines) to PHP physicians and practitioners.
  • A Healthy Heart action plan that the member can complete with their physician.
  • Free access to a cardiovascular RN case manager to assist the member with questions about heart disease prevention and management.
