  Right to Complain about Quality of Service or Care

If you are unhappy with the service or care you receive while a PHPMM member, we want to know about your experience.  You may contact Customer Service through our secure general inquiry form, write us a letter, or call us at 800.832.9186. We will investigate your situation and respond to your concern quickly. If you are not satisfied with our response, you may be able to file an appeal, called a grievance. Customer Service can help you file a grievance, if necessary.
