HealthWeb® is a web-based collection of tools which provide access to claims, eligibility, benefit, and health information. Visit any time of day to request, update, change, or provide your personal information. All forms are secure; so please be assured that PHP is dedicated to keeping your personal information safe.

Request a Copy of the Certificate of Coverage

PHP members can access benefit information through our Member Packet Portal. This portal allows online access to member materials, including the PHP Certificate of Coverage, Member Handbook, and Provider Directory. As a member, you can either view or print the documents for your particular benefit plan. You may also request that hard copies be mailed to your home. to access the Member Packet Portal.
