Physicians Health Plan offers a wide variety of benefit designs. 
 

• The HMO product requires members to seek services with a participating physician or facility.
• The PHP Plus product gives members the opportunity to seek services outside our network.  The PHP Plus product does have an out-of-network deductible and some services may not be covered out-of-network.
• PHP's PPO product provides statewide and national coverage through partnering networks.
• PHP also provides TPA services to those large employers seeking to self-insure.
