Fill out the Pharmacy PA Form and fax back to our localized Pharmacy Department at 517.364.8413. ThePharmacy PA Form is in Adobe Acrobat format and may be printed to be filled out by hand then faxed back to us. Be sure to include the Physician's signature before faxing.
