This site is designed to provide you with information you need to know when treating a PHP member in your office or facility. It contains information regarding PHP and PHP FamilyCare products, medical management and quality improvement programs, billing and claim procedures, ID cards and eligibility verification, pharmacy benefits, preferred drug lists and mail order perscriptions. Billing information can be found in the PHP Provider Manual as well as the United Behavioral Health (UBH) Provider Manual The procedures and descriptions outlined in the manuals is intended to cover most contracting situations you and your office staff will encounter.

The manuals should not be used for determining coverage in any particular case. Coverage may be determined by our Customer Service Department as noted in the "How to Reach Us" section of either manual. While we have tried to make these manuals complete, they are not intended to include every administrative policy or procedure.

Note: PHP FamilyCare follows MSA billing guidelines. Participating and non-participating providers with PHP FamilyCare will be reimbursed according to the State of Michigan Medicaid Fee Schedule.
