Prescription Drug List
The Prescription Drug List (PDL) includes representation from each therapeutic class. Your Certificate of Coverage defines your actual benefit coverage and may exclude coverage for certain drugs listed in the PDL.

SPD Tool
PHPMM provides employers with a SPD Tool to assist in creating your Summary Plan Description. Once you have entered the required information, you will receive an electronic copy of your SPD.
